The completed form will be sent through to office automatically once submission has been completed
- you have selected correct availability type
- your elected dates correspond to actual period you are entering
- elect at least one option for each day of the week (even if Not Available)
- clear your cache/browser after each submission of hours provided
(i.e between weeks or resubmissions)
